APPLICATION TO THE DATA CONTROLLER AND INFORMATION FORM UNDER LAW NO.6698
GENERAL EXPLANATIONS
In the Law No. 6698 on the Protection of Personal Data (“Law No. 6698”), the persons specified as the person concerned (hereinafter referred to as the “Applicant”) are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the Law No. 6698.
13/1 of Law No. 6698. In accordance with the article, the applications to be made to our Company, which is the data controller, regarding these rights, must be submitted to us in writing or by other methods determined by the Personal Data Protection Board ("Board").
It is important that the personal data we hold about you are accurate and up to date. Therefore, please notify us when there is a change in your personal data. Regarding yourself in accordance with the Law No. 6698 and the relevant legislation;

Although it has been processed in accordance with the provisions of the law and other relevant laws, if the reasons for its processing disappear, to request the deletion or destruction of your personal data and to request the third parties to whom your personal data has been transferred,
Object to the occurrence of an unfavorable result by analyzing your processed data exclusively through automated systems,
In the event that your personal data is damaged due to unlawful processing, we inform you that you have the right to demand the compensation of the damage.
In this context, the applications to be made to our Company in "written" form are printed out and written "Personal Data Protection Law Information Request" on the envelope or on the subject;

REQUESTS OF THE APPLICANT
According to Article 11 of Law No. 6698, the rights you can request from our company, which is the data controller, are specified. Your applications submitted to us are evaluated within our company according to the rights you demand, and 13/2 of the Law numbered 6698. According to the article, your request will be answered as soon as possible from the date of receipt of your request and within 30 (thirty) days at the latest, depending on the nature of the request. 13/3 of Law No. 6698. In accordance with the article, our reply will be sent to you in written or electronic form.
